Every five years, the Florida Deparment of Health in Charlotte County completes the Community Health and Needs Assessment (CHNA), which is then used to produce and inform a Community Health Improvement Plan (CHIP). The purpose of the CHNA is to review and analyze relevant data to provide invaluable information on the overall health and well-being of our community. However, the 2025-2029 CHA is composed of a much robust analysis that considered the economic, social, environmental, behavioral, and educational factors, which all invariably impact individual and population level health outcomes. Healthy Charlotte also works on a CHIP as part of its commitment to the continued assessment and development of community-based interventions to improve the health and well-being of all residents.
